For my form I'm required to print off a hard copy. the PDF file is missing some of the required fields!!!! whats happening?Page URL:

Hi millerstrust, I have checked your form and I couldn't replicate this bug again. Can you tell me which fields and which browser ?

can you check my settings?? Im still not getting the entire form like above, Im using 3 different machings, running different o/s and browsers..
I can only imagine that somethigs changed and its not showing the fields.

I have tested it on Windows9/Mac OS x and IE7-8-9, Chrome and Firefox.

Are you receiving the same PDFs on your Dropbox account?

yes, the pdf's in the dropbox are exactly the same as the ones i download off the site, missing the users name and old address and second name fields.

Try updating your adobe reader. I tested it and its working fine on my end

youve not understood my problem... its the pdf of the submissions thats missing fields.

Once again please accept our sincere apologies for the inconveniences you are facing.
I can reproduce the problem with your form, but submission PDFs look fine on the same cloned form. As a temporary workaround you may clone your form and use it, I hope it will not be affected by mentioned bug.
