Headers show on submission but not on PDF

Here is my form https://form.jotform.com/60406038491149.  When I go to the submission the headers show up.  But when I click "Get PDF" and download the PDF, the headers are missing.  I have already clicked the gear icon and checked "Show headers and texts".
