I'm having trouble with the Audioboo.fm widget. The wrong files keep loading up in the form, the audio files get reordered around the page, and sometimes the widget doesn't appear at all. (We had the same problem with the Soundcloud widget).
If you can access our '¡Practica tu pronunciación! Connected Speech' form, you will see the issue.We have also had issues with the voice recorder. People we have sent the form to have been unable to save their audio files as the saving never stops...

Currently, I didn't see the "Audioboo.fm" or "Soundcloud" widget on the form you have shared. Maybe, the problem you mentioned has occurred again. However, I didn't replicate the same issue with you on my tries, there is no disappearing or wrong file issue.
In this circumstances, I get suspicious about the form and browser caches, so I cleaned your account form caches and please, remove your browser caches on your side and try to add those widgets again.

Thanks for the response. We have cleared our caches, but we are still getting the same issue.
I have attached two screenshots: one is the form editor itself (next to Grabación 1, 2 etc. you should be able to see the Audioboo.fm widget, but you can't). The second screenshot is of the live form where the wrong audio files are in each widget (the numbers should read CS-01-01, CS-01-02, CS-01-03).

Dean, I got the same results with you on my first run; but after refreshing the links on the "AudioBoom" widgets, I can see the correct audio file names on the live version.
Please, check it from the below:
At this stage, I suggest you to refresh all of the widgets' URL fields with the correct links and save your form. With that one, the problem is going to be resolved.

Thanks for getting back to me so quickly again. Refreshing the widgets did fix it in the form editor, but what shows on the live version is different...
This is an issue we had last week where the editor was showing the right audio files in the right place, but the live version (and also the preview) had some of the audio files jumbled up.
I've attached a screenshot from the editor and one from the live view. (From page 3 of the form).
