I've tried contacting you last week but haven't heard back. Please let me know how I can edit my form because it doesn't appear on my screen and I've been told my username isn't valid.

Please register for a 'Free' JotForm account using the email address you entered making this post and the form should automatically get transfered over to your new JotForm account.
Let us know if you run into any problems following that process.
