"Terms and Conditions" widget does not display on source code embedded form

The terms and conditions field does not display on my website.  Only the check box and asterisk.  Please advise on how to fix this.

I took a look at the page that you have linked above and I see that the form is embedded using the source code embed embed code.

That is the reason why the I agree to terms & conditions.* is not shown with the link to http://www.thedanceministrymanual.com/terms-and-conditions.html

To resolve this you could simply switch your embed code to iframe embed. You can see here how to get the same: How to get the jotform iFrame Code

Once you do the form will work just as it does in the preview for you, but it will be shown within your website.
