"form not found" Disabled - Please Help ASAP

I need assistance with my form.  I am losing money because customers are calling me as they are unable to complete the reservation form on my website.

It is saying that my form is disable and there is an error message "Form Not Found, "The form is disabled"

Please help me to fix this ASAP!!
