I cannot find Mini Contact Form with CSS3

    I have helped several of my friends setup their jotform using the template "Mini Contact Form with CSS3" like in this case:  https://form.jotform.com/60784448376165

    BUT .. I can no longer find the template "Mini Contact Form with CSS3" 

    Can you help me find this template quickly and easily?  

    NOTE:  Below you request a URL .. so I will enter the one that we previously made for Eugenia .. but my question and dilemma is finding this template again for a new friend. ok?
