How do I create a question with the option to voice record as the answer not text response.

It is possible to record voice to your JotForm by using Voice Recorder widget. I see that you have already added the widget to your form. Once the form is submitted normally, the recorded voice shall be attached to your JotForm submission. However, I see that there is no Submit button on the form to submit. Please add the submit button to your form and see if the widget works for your requirement.
