Please help! Our main form is not working!! Says we've reached our "quota"?

We locked in the $9.95 per month fee for 1,000 forms per month and today we can't use our form,  it says we've reached our quota.  We have NOT submitted 1,000 forms this month.  We need this resolved immediately!!!!
