I just checked our website (http://www.modation.nl) and wanted to see if the contact form is still working.
I filled in the fields of the contact form and was send to http://www.jotformeu.com/submit.php. But on this page no message (unique code) comes up which I have to fill in. This prevents me from submitting my form, which not only I but also our clients will have.
Can you clarify this problem and come to a quick solution.
When pulling up the form using the direct JotForm url the captcha do not appear and the redirection works. Can you please login to your JotForm account, save the form and get the embed code and re embed it to your website.

Embedding the code didn't solve my problem. On the Jotform code we built our own form (codes you can see on our website). This worked for a long time, so I think maybe it has to do something with the ban of jotform.com? I did also try to change it into jotformeu.com, but this won't solve it.
Can you look into our codes and see what goes wrong? The only thing not working is not coming up any figures which you have to fill in before submitting your form.
