Cannot create PDF form

I am having trouble creating the pdf form..... the page just gets hung up Please advise. 

I could replicate the same situation, but I believe it is connected with the large amount of Text fields that are present in the form. 

I created a test form and could create a PDF version, so the problem is specific to that form

I have created a PDF version of your form and I've sent it to the email associated with the Jotform account.

You should be able to convert your online form to Fillable PDF form without any issues. However sometimes if he form is larger it will require more time for the Fillable PDF form to be created. Sometimes very large forms can not be converted to PDF forms. 

Widgets and conditions are not supported in PDF forms. More about Fillable PDF forms you can find inside of this guide:
