In the preview mode, the field shows up (Private Teacher's email or phone number), but when I embed the form, the field does not show on the linked form. I've cleared cache, even re-embedded the form after saving multiple times, but still it persists.

A ticket has already been opened and forwarded to our developers regarding this issue, it appears that the embed link http://form.jotform.com/form/20223957011 is not updated with the changes. Please use the following url http://www.jotform.com/form/20223957011 in your embed code instead.

After making the change suggested by my colleague, can you now see the field that was missing? A few users are reporting this failure of their forms to display updates when they use the http://form.jotform.com/ version as opposed to http://www.jotform.com/.

Yes - the field now appears. I don't know if this helps your troubleshooting, but the field that was not showing has been there for weeks, it was not new.
I appreciate your quick response! I have had two issues using JotForm and both times the support team was wonderful.

And that extra bit of information about the field not showing for three weeks definitely helps us with troubleshooting this issue. To my knowledge, it shouldn't take more than a day or so for forms at the form.jotform URLs to update with changes made in the form builder. I'll pass the info on to our developers.
