Hi Beril, unfortunately the problem still persists.. I have tried with both Internet Explorer and Chrome.

We are deeply sorry for the inconvenience. I had tested before informing you. It was working properly. It seems that issue has been realised again. I've forwarded hat issue to our one of developers. We will inform you via this thread as soon as possible.

Our developer fixed that issue so quickly. I've also tested it and it is working without any issue.

I'm not sure why it is still not working on my end. :(
I have also tried the other form with the same results
Could it be a network related issue? I will try again from my PC at home and let you know if the issue still exists.

I have checked this from my side, and the PDF generation worked correctly with my test forms:
I have also logged into your account and tried generating a PDF from your form named "Achievement Awards Entry Form 2016", and it too worked without any issues:
It could be a network issue, or even more likely a caching issue. Can you please try generating a fillable PDF of your form from a different web browser, or by using the Private / Incognito window of your browser?
As far as we can see, the generation of PDF forms seems to be working correctly now. We look forward to hearing how it goes.

Thank you so much, I am not sure how it was fixed but it seems to be all working now.. very strange!
I tried on my PC at home and that worked, then I tried the Incognito window and that worked, so I thought I would try it in a normal window at work and that all of a sudden worked also!
