Is there a way to disable the pop-out so the form can only be viewed within the iframe?

If you embed a PDF with a direct iFrame method, it should not allow any pop-up options. Please see the next guide.

How to Display a PDF Document On Your Form

Note, that most of the browser PDF viewers provide a Save/Download option.

Thank you. I saw that yesterday. I researched this for hours before asking. Best I could do was embed as iframe with toolbar="0" but it's not displaying correctly. If it's not possible to disable the "pop-out" option, please let me know. 

Seems like you are using an iFrame code to embed the PDF file in your form, although this could work I would suggest you to use our iFrame widget, it could work as well.

On the Form Builder you only should add the widget to your form: 

Then you only need to paste the URL where is stored your PDF file:
