I presume you are referring to the registered account with username copyitusa? I have answered the inquiry on this thread http://www.jotform.com/answers/802832. However, you'll need to log in to your account to view it if you're using a different email address aside from the one in your JotForm account.
Regardless, that account should now be able to receive the email reset password. Let us know if you are still having problems on this.
