how can i convert my word doc to a fill in form?

    The patient information forms for our office are quite lengthy and done in word format. I've started to reproduce them with the jotform templates, but it's taking forever and there are some things I can't seem to replicate. If I upload the forms, is there a way to convert them to a fill in form without having to recreate them?

    Unfortunately, you can't import a word doc into Jotform. A possible solution would be to create a fillable PDF of your document.

    I found a nice Youtube tutorial that might be helpful.

    For this, you will need to first print your doc into a PDF. With Acrobat PDF Professional, you will be able to convert it into a fill-able PDF.

    Once you have the fill-able PDF, you may use the following link to import the PDF into your JotForm account:
