I created a form back in October, but I can't find it in MY FORMS anymore. I can still see it on my web page and receive submissions.
How can I find it in MY FORMS and edit it if necessary?

I have checked the form and found out that it was created last October under a Guest Account that is why you can't find it in your frewsch account.
I have moved the form to your frewsch account and its title is Website Design Request Form.
