I have an application that I have created already using word. I would like to copy the format and layout to jotform. Is that possible?

Here is an example of our application and all the information I need on our form. Any help would be greatly appreciated!

Unfortunately, it is not possible to import a Word document to JotForm and it is required to build the form in JotForm again. Please be noted that building a form using JotForm is easy by using the intuitive drag and drop fields. Once you have the form ready, you may design your custom theme by using our Form Designer or you may use one of the themes readily available at our theme-store.
