My form is extremely slow

My form is loading extremely slowly.

In addition, once it loads, there is a 2-4 sec delay between clicking in a checkbox/radio button and the box filling.

We work in the medical field, and patients are very frustrated by the length of their visits due to this porblem.

Please help!

I see that your form is having about 400 fields in total and 156 conditions which need to be considered as a long form. Though JotForm builder allows to have unlimited fields on a single form, the browsers may not be able to render the form due to their limitations with the memory, processing speed, etc. Sometimes, this might also result in freezing the browser as well.

Since you have different sections on the form, you may consider breaking the form to separate forms to reduce the count of the fields on the form. You may redirect to the following sections of the form by using the Custom URL method in the Thank you page.

You'll also need to pass the basic information of the patient to the following form in order to identify the submission to match with the other form submission.
