I did a test submission on your form (https://www.jotform.com/21586009634152) and it appeared in the mail logs. It is showing that it was sent successfully.
I checked the configuration of your email notification and I can see that it set up properly. Please do check your spam or junk folders. You can also try white-listing JotMail IP addresses. You can also use your own email address. Here's a guide: How-to-Setup-SMTP-for-a-Form
