Do you mean to say that your form's custom URL shows 404 error message?

I did check the URL and I am unable to replicate the issue you are having. It does display the form correctly and I was also able to submit it successfully. Please check the screenshot below:

I get the error when the form is custom linked - when not custom linked it works fine

Can you please provide us steps on how to reproduce the problem?

So far like my colleague Ashwin, I could not reproduce the problem when I submit the form using your custom URL https://form.jotformeu.com/Mobileonix/pieta
