when form is filled out only email@example.com is receiving the email request.
Checked bulk/junk. Checked with hosting company. checked webmail.
user account: firstname.lastname@example.org
I checked our mail logs for the email address email@example.com and firstname.lastname@example.org. It appears that the previous emails failed to send, but the most recent submissions (including my test submission) were sent successfully. Please confirm on your end if the email notification was received.
