How do I get my Form from Word to Jotform?

Unfortunately you cannot import form template from Word documents.

You will need to use the Form Builder to manually create your form. You can use the Word format as your guide instead while creating your JotForm.

You can also use the Form Gallery to search for template form that could already match your form requirements.
