How to allow users to read, sign and submit a payment with JotForm?

    We are trying to create an online registration form for our preschool summer camp. The parents need to read and sign a disclaimer form and submit this form with their registration payment.

    Is it possible to have them read the form online within the Jotform and somehow sign it?

    Is it possible for us, the office, to be able to then take that signed form and somehow print it out with proof of their signature so we can put it in their file that the camp counselors then keep with them during camp hours?

    I'm not sure what are you referring with "read the form online", I think that you are referring to read some text that you will add to your form.

    Regarding to sign your form I think that you should use the E-Signature widget: http://widgets.jotform.com/widget/signature

    This will allow your users to sign before to send their form.
