If possible, I would also like to have the form submissions sent to my email address.

Ok, I have figured out my own question but developed another.
My form is emailing the results but only 3 fields out of 8. It seems it's only submitting my text field results and none of my check box results. Is there something I am missing to enable these other results?

I have tested your form and everything is working perfectly. Please note that only selected answers from checkbox field will be included on the notification email message.
With this options, I only select Option a & Option b, so in the mail message it will only contain Option a & Option b. I hope this clarifies this issue.
Also note that, your checkbox fields aren't set as required so please keep in mind that if your form users didn't select any from the checkbox options, you will then received a blank answer as a result.

After I had posted this question, I closed down jotform and later when I reopened it, it was working fine. Must have been some glitch going on at first. Thank you for checking and confirming!
