el fomulario en el que estoy trabajando se ve como que no hubiera nada no esta habilitado

lo que pasa es que estoy utilizando un formulario q desarrollamos para una encuesta y esta como bloqueado y no me sale ningun envio como que no existieran ninguno,,,,es un problema grande por que estan haciendo las encuestas y se quedaron sin el formulario
