Hello!
I use the appointment slot widget as a volunteer sign up form. I need to separate the questions on the submissions report. Right now the report shows all of the questions asked or filled out from the widget in a single column. Is there a way to separate them so I can easily view who has signed up for each position? The next problem would then be if someone signed up for multiple positions. Ideally it would be great to have the name of the person signed up under each question. example:
check-in bookstore dinner set-up
Mary Dave Mary
Lisa Tom Jenny
Sherri
Dan
Anyway to work this? Thanks for your time and help!

You can create excel report of your submission. Then your data for each submission will be shown in columns.
I have cloned your form and made few test submissions. Then I have created Excel report. My Excel report looks like this.

Thanks for the answer, but its not really what I'm looking for. In the excel export each submission is its own row. If there is more than one check or answer in the same submission it is showing up in the same column "please click here to sign up for available volunteer positions".
I would like to break apart each of the questions into its own column as each question like first name, last name, email and phone number is its own column. So I can see who signed up for each question.

Unfortunately, we do not have built-in feature to achieve this. However, you may export the submissions data to Excel and have the data separated to columns. Since the data of Appointment Slots widget is having one entry per line i.e., separated by Enter Key, we can use this as a delimiter to separate the data to columns. Please see the screencast below:
