I've uploaded a background image to my form which shows in the designer, but when I preview the form, it is a completely white form and then the thank you page has the image i uploaded for the form background. can you help?

The image is not loading on the form because it is being served with an invalid SSL certificate. This means that the image can only be loaded over the plain HTTP protocol, and since the form is using https, the image cannot be seen:

But if you load the form using http://www.districtdanceco.com/wp-content/uploads/2015/10/dancer3-e1443743452190.jpg then you will be able to see the image in the form.

To resolve the issue, I would suggest hosting the image in a server that supports secure connection. Or you can also try creating a separate form in jotform where you can upload the image, then go to the submissions page and use that link as your background image. Simply Right click on the link and select "copy link address" to copy the secure (https) url.
