Seen to be we are loosing business with this system!
please help before we change this issues,

Could you provide us with more information on the issue that you are experiencing with your JotForm? I've tried submitting the form on your web page and I was able to submit it without any issues. Please see the screenshot below:
However, I notice that the form on the webpage is associated with a different account sismercado. If you are referring to your form Restaurant Reservation Form in your account carambola, I see that account is in Dormant status. I've now activated the account and you should be able to receive the submissions for this form as well.
