I think your forms were disabled because you are asking for Credit Card information on them something that is not allowed in JotForm, you can not ask and save Credit Card information, you only can use your forms to sell products, subscriptions or collect donations and for these purposes you must use our payment integrations: http://www.jotform.com/help/47-Order-Form-Basics.
I have removed those fields from your form in order to active your account again, please follow this advice and read our terms as well: http://www.jotform.com/terms/.
Otherwise, your account could be suspended permanently.
