Our sign-up forms through JotForm include a dynamic question (workshop/recital date). As users sign up, available slots are removed from the options. Since the site went down and since it came back up, all of the slots are showing as available despite previous submissions. How can I fix this?

I couldn't find this form. Can you provide the URL for the form and let me know which question is this on the form?
