We need help and are extremely close to moving all of our client websites over to competitor form submission websites as the customer service has been terrible. We have emailed email@example.com 4 times now for answers and nothing. As the owner of a marketing firm and multiple brands, I have to say from an unbiased point of view, that this is not the way to provide service... through a forum? Thats a joke.
Please find the attached screenshot of how the forum from fitphysicaltherapy.com is being manipulated and appearing like a amateur put this together.
Our client is not happy, and I must say, we share the same emotion.
Please prove that you care about business. If we do not hear back about this issue by tomorrow, we are transferring all of our clients new and old to another platform.

We sincerely apologize for the inconvenience. I checked the pdf version of the submission seen in your screenshot, and it appears that the issue has been resolved. The pdf I saw appears to have been formatted correctly.
If you do encounter the issue again, please try resetting the form to its default design.
