I can't seem to find a questionnaire that was created this month.

I need to add a questionnaire to the form, but it's not showing up in My Forms. I can only find the filled out forms (those who have completed the form online).

Do you mean that your form is not showing up in your My Forms but you are able to find the filled in forms which I am under the impression is from you receiving notifications about them perhaps?

I looked in your jotform account and found the form you mentioned above in your account.

Currently your account is active and I didn't find any issues with it on my end but I've cleared your form cache to refresh it for you.

Can you try checking again now to see if it's still missing or not?

If it is then please log out then log back in again to try checking it again to see if the form will show up in your My Forms for you now then let us know if you're still having an issue with it or not.
