Our Account has been suspended. You believe we have been phishing. We need to get this rectified please.

I have checked your forms and in most of them you are asking for a password change, this is something not allowed in JotFrom, this is why our account has been suspended, please read our terms on this link: http://www.jotform.com/terms/.
In order to activate your account again, I had to remove those fields, otherwise you will not be able to log in again, please follow this advice and avoid to use words such as user, password, username or ask for Credit Card information, if you are still asking for this kind of information your account may be deleted.
