    Browser Compatibility issue - Checkboxes and Columns not showing in IE

    Hi there,  

    I'm having a problem that only seems to be related to IE.

    In Chrome, FF, and Safari the answers for the checkbox questions for "Conference Room Requested" and "Set-up" are appropriately spaced out horizontally by column, but in IE, they appear vertically one stacked on top of another.  Any idea why this is and what i can do about it?

    You have inserted the form codes on a (literally) blank html page. You must have the basic html codes (w /doctype definition) in the page for all formatting to work properly.

    Alternatively, to make things easier and less problematic, simply re-embed your forms using iFrame method.
