It is a new form called Harvey's Terms and Conditions, which I have embedded in my website which is working well. However, I also need to convert it to a printable PDF. When I try this and select the conversion to have page breaks it only produces two pages and the first page is blank. How can I get this form to produce a printable PDF?

I was unable to reproduce the problem due to the issue with PDF form creation problem on my end. I will escalate the issue to our L2 support.
Please try creating the PDF form again on your end, and let us know if it works for you.

Sorry for the inconvenience. The fillable PDF creator is still not working. My colleague Nik already escalated this issue to our developers. We'll let you know on this thread once an update is available.

The PDF is now created but the original problem remains, ie, it only produces two pages ( there should be more) and the first page is blank.

Try to uncheck 'include page breaks' when you click on Create PDF button:
I did a test with your form and it worked fine.

PDF Creation works when I uncheck the "Include page breaks" box. This produces a PDF of one continuous sheet. This is fine but when I try to printed it only prints one A4 page. How do I get it to print the entire document?

Do you mean to say that the print result is cut off, not all texts are in the print result? If so, I think that is because of the size of the document. The printing preferences can only print the visible texts. Please try doing the following changes in the Print Options:
The screenshot is through Adobe Acrobat Reader.
In the screenshot, you'll see how to change the print/page size.
Lastly, you don't have any fields in the form so why not create the PDF manually using any other tool? I mean, our PDF creator has limited options while using other PDF creator allows you to customize the output. Of course, if you have some fields in the form, you can't create it with any other tool.
