How to generate invoices from forms?

I want to create an invoice using jotform.com to add as a page on my website like this one, http://www.stroucoonline.com/employee-corner.html

 tried but no-go. HELP

How would you like to have the invoice be generated? Do you want it to appear within the form upon fill up? Or, do you want it sent via email to your submitters? Also, the invoice you would like to produce, is it going to be generated out from the form embedded on the website you have provided? Please give  us more information so we can provide you appropriate instructions on how to do it, if it is possible.

I'm sorry, I might have missed it. But, after re-reading your request, here's what I understand now:

First, you would want a form similar to the form on your website, correct?

Second, you would want to have that form be filled up and act like an invoice, is that also correct?

If that's that case, may we know where will the data that would be generated be coming from?
