All of my forms have been disabled / not found ?

I have checked your form and some of it are directly asking for username and password. Please be reminded that we have an Anti-Phishing system that monitors all forms 24/7 and marks the one suspected of phishing activity that violates our Terms of Use.
I will enable your account for now, but make sure to remove the fields that clearly asked for login credentials on some of your form.
