Upon clicking view submissions on this form, I am only prompt with the date of submission. No other information is present in the window. This used to work fine, but I find it unusual as even previous submissions which used to include all information are now showing as blank.

I did check your account and found that you enabled the option to store data in Europe Datacenter. Please check the screenshot below:
You will need to ensure that you sign in to your JotForm account using EU domain: http://eu.jotform.com/
Please sign in to your JotForm account using above EU domain and you should be able to see your submission data.
