    How do I add the form on my web page?

    I used to put the form address in my web page to have it accessed from there but it is taking me to jotform instead.

    You can embed the form so that it looks like it is part of the website, then you need to add it to your webpage. Please refer to this guide: Adding-a-Form-to-a-Web-Page
