Why was my form disabled?

    I have a form that has been disabled but I don't know why. It isn't collecting any personal data. Its just an employee status change form through which our managers submit changes regarding an employee's pay rate, store location, or position. I wasn't aware that any of this information was a problem.

    I checked your forms and they do look to be within our terms of use.  Our system is much more strict on newer accounts especially when they ask for bank accounts details.  Though the details you are requesting are acceptable.
