I had my provider install our SSL certificate from Comodo. I'm having trouble trying to load a secure page on my site with a secure JotForm embedded.

Load this page and click the link to load it as https. It all looks good. But if you follow the link from temp2 to temp1, where the JotForm form is embedded, you'll see that the secuity icon says "this webpage does not provide security information".
I'm obviously new to this so there must be something I've missed.

The form itself is using https, however the image in your form is not:
http://tbsmahopac.org/images/TBSLogo.jpg
Replacing that image in your form with one hosted from a secure source should clear that error. Other than that, I do not see any further errors.
You can also try replacing your current embed code with your forms iFrame code:
http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
That will ensure there is no conflict between your form and the webpage. If there are any errors I am not seeing on my end, that will rule out any issues between your form and the page itself.

I replaced the image from one hosted on my site, unsecured, to an image uploaded to JotForm.
No More Problem!
So glad it was something simple and I thank you for chasing it down for me.
