How do I rename the 'Course Instructor Feedback Form' in the notification that I copied

    I am getting an email with the Course Instructor Feedback Form because I modeled from it to create my Broadcast Request Form.  When it comes into my email, it is mislabeled.  Can you reply with how I can change that label?

    I understand that you want to change the title on the notification email. You may change the title by editing the notification from the email setup wizard. Please see the screencast below that can help you in changing the title on the notification email.
