How can I create a two column form?

    Hi,

    I'm not sure why there's no respond in my reply on the recent support ticket I opened.

    How can I achieve this form layout?

    So far, the custom css sent to me didn't solve my issue.

    You can see the text area and captcha is not positioned on the right side (2nd column). I hope you can help me. This is urgent.
