I have a form that has been created for donations and I was wondering if there is a checkbox option I could insert that would make the payment a monthly subscription payment. Is that possible?Page URL:

If there isn't a way to have this functionality. Is there a way the form can push a new form in when selecting a payment type button? A similiar form would be this one:
Could something like this be done? Essentially I create a One Time Payment and Recurring selection and create a condition that once recurring is selected we're directed to a new form or maybe a new form takes the place of the current one. Thoughts?

Unfortunately, we don't allow using multiple payment integrations on one form. However, you can create a form for each payment type, and embed them in your main form via iFrame embedding. You would then hide both forms and create a condition that will determine what form will be shown on your main form when the user clicks on a radio button.
Please take a look at the following guide on how to create multiple payment forms.

Is there a conditional element that will select the coinciding subscription amount with the amount the user selects at the top of this form? t Or maybe it's just s styling option on the actual subscription numbers. What would be the best option?
