Getting: Form not found - This form is disabled error

Did anything change with your free subscriptions?  I am getting this error "Form not found - This form is disabled" 

I checked and all forms are like that

All forms are enabled

Nowhere near my quota and have 0 for financial transactions.

After reading through several others, it appears it may be on hold for a assumed phishing flag because I request a "username" in some of the fields.  I will update to just say name and see if that works.  

I removed everything that said "username" from every form, but it still shows the same error message.  No clue what do to next except wait on admins to take a look.  

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on Phishing forms.

I have verified your forms and they seem to be normal and clean since you have now removed the fields username. I have activated your account and you should be able to see your forms now.
