    How do I find which account I have a form saved in?

    I cannot remember my username and password to access http://form.jotform.us/form/43064568389164 this form.  How can I find it?

    You can paste the form ID in the search text box in My Forms page, if the form was created in your account then it should be shown when doing this: 

    Otherwise I think that there could not be other way to know the account username where it was created; however, as you lost your username and password I would suggest you to reset it, I will provide you with the username where that form was created: abqhomesearch

    An email will be sent to the email address related to that account, that I noticed it is the same than the one you are using on the account of this post, the email with the reset instructions will be sent to this email address: s***t@a**********ch.com
