Unable to change form name?

Hi,

The form works perfectly fine, my issue is as per the attachment is when I receive a submission the subject line on the email reflect to the form title however on the actual form displayed below it has a different name "Training Evaluation Form"

I have tried to search for this in many places unable to find to make changes can you please help.

Once on the edit mode of your form, click on "Emails", and you will see the Compose Email, there you can change the title:

Once changed, click on Finish and save your form. Let us know if you need more help.
