Form fields in the wrong order

Hi Guys,

What am I doing wrong?  The shuffle option is turned off - http://snag.gy/gweZY.jpg

You are looking a the wrong field, you have two fields:

The first field titled does not show, because it is conditioned to show only if a specific option in another field is selected:

But no selection can be done, since that field is hidden by default:

So, yo may click on "Show Field", so that field displays when form is loaded, and if "Yes" option is selected, then your other field "Do any borrowers have a poor credit history" will show up.
