My form has been disabled

Hello, the only form I have has been disabled. I do not understand why. I use it properly and my page is not fraud. Please check the problem and reply me as soon, as possible, thank you :)

Our phishing filter in place had wrongly categorized your form as a phishing form because it was not able to detect a proper Title/Name on the form.

I have corrected the mistake and have now reactivated the account and form.
